The patient's history is accomplished in two phases, the patient generated medical history (PGMH) and physician editing. In the PGMH module, the patient developS his or her medical history. This saves the health care provider a tremendous amount of time. The health care provider reviews and edits the patient's history through TURBOHX's edit module.

The History Taking Process

PGMH is a unique expert system that selects appropriate follow-up questions from over 11,000 questions. Its questions are gender specific. Thus your patients, their proxies, or one of your low level staff can do PGMH at home or in your office and create a comprehensive medical history.

HX asks the questions. Only after the patient presses the Press when finished button does TURBOHX ask the next question.

  • When describing areas of concern, the patient simply clicks on an illustration indicating the area of concern. TURBOHX translates the location into words, such as "right upper quadrant."

  • TURBOHX allows the patient to reveal his or her fears and concerns. And studies indicate that patients reveal more personal information to a computer than to a person.

 

After the patient completes the history, the information is transformed into a comprehensive history report. The health care provider reviews and can modify the history. Clicking on a link in the left frame will add or remove that item from the main frame, where the history text is located. Thus the health care provider can determine which items will appear in the history. These items include various complaints, such as abdominal pain, etc. or categories such as social history, hospitalization, etc. The health care provider can also modify the specific text by clicking on it. Then the questions and responses related to that text appear below with the appropriate responses filled in. The health care provider simply changes those responses, which in turn alters the report text.

How is the Patient Generated Medical History different from present history taking?

Up to now, there have been two basic methods of taking a medical history: the interview process and the patient questionnaire. In the interview process, the health professional asks a question and based on the patient's response, asks a series of follow-up questions. This method can produce a comprehensive history provided the health professional has the time to ask the appropriate follow-up questions. The problem with this method is that it is very time consuming and therefore costly. When the health professional has limited time or forgets to ask all the pertinent questions, then the quality of the history suffers.

The patient questionnaire is filled out by the patient; the health professional is not involved in the process. However, the questionnaire produces a very superficial and poorly organized history. Usually the patient will check the symptom that he or she has. There is no in-depth exploration of those symptoms. For example, if a patient indicates he or she has diarrhea, the additional dozen follow-up questions about diarrhea are not asked. If all the appropriate follow-up questions for each symptom and disease were on the questionnaire, the questionnaire would be hundreds of pages long. Finally, the completed questionnaire provides the physician with a disorganized list of checks scattered throughout the questionnaire.

MNS PGMH combines the best aspects of history taking methods into a revolutionary new paradigm. The patient completes the medical history prior to seeing the physician. The questions are self-explanatory and can be answered by most people. Unlike a questionnaire, MNS PGMH allows the patient to tell his or her story, fears, and concerns in his or her words, thus giving you better insight into the patient. When a patient indicates he or she has a symptom or medical condition, MNS PGMH will ask the appropriate follow-up questions, just like the health care professional would. On the return visit, the patient reviews his or her history in TURBOHX. By themselves or with help, they review and can edit the history as described above. New complaints can be made by the patient. These are recorded through the PGMH. After the patient has completed his or her review, the health care provider reviews and edits the history throughTURBOHX, as described above.
