All patients in the practice are automatically included in the Patient Registries.  There are two types of registries.  

  1. Wellness Registries.   These include all the patients with given demographics who are due for a given test or procedure.   These are the same guidelines as found in the point of care Guideline Wizard wellness guidelines.
  2. Disease Registries.  All patients with a given disease listed in a given registry with the appropriate disease management and disease management medication guidelines.  These are the same guidelines as found in the point of care Guideline Wizard disease management guidelines.  The current diseases in the registry are diabetes, hypertension, heart failure, asthma, and coronary artery disease.  New diseases can easily be added to the registry.

The Patient Registries are fully integrated with EncounterSuite and the Guideline Wizard.

There is no redundant data entry.

The Patient Registries are not a superimposed module requiring redundant patient data and diagnosis entry.  The registries are fully integrated with the Patient Encounter Interface and the Point of Care Guideline Wizard.  When a patient is added to the system, the appropriate wellness guidelines (see Guideline Wizard) are created based on the patient's demographics.  When you diagnose a patient with a medical problem in the Patient Encounter Interface, the patient is automatically added to the appropriate to the disease registry. 

How the Patient Registries Work

The Wellness Registries list the patients who need testing and procedures. The disease registry displays all the patients with a given disease and the status of the disease management compliance.  The registry guidelines work the same way as they do in the point of care Guideline Wizard.
 

Fairer, More Sophisticated Audits 

Sometimes a guideline should not be applied to a given patient.  With some systems this can be hard to document.  With the Patient Registries, you can easily explain your thought process and can even set a duration that the guideline should not be applied in the guideline.  This feature enables a fairer audit.

1. The Not Applicable link is clicked, opening the Not Applicable window.
2. The reasoning (in red) why the guideline should not applied to the patient is documented and submitted.
3. The guideline is shown as "met" and the reason why the guideline is not applicable to this patient is displayed.

 

Because the status of an order appears in the guideline, patient non-compliance is documented ... an order was made but the patient did not get the test done.

Guidelines can be edited, inactivated, and created.
