• Patient Registries.  Patient registries contain all the patients in your practice.  Wellness registries list all the patients needing evidenced-based tests, procedures, and vaccinations.  Disease registries list the patients in the practice with a given disease and all the evidenced-based disease management guidelines for each patient.

• Inbox.  All orders go into the inbox and remain there until they are completed.  An order is never lost!
• Tracking Your Chronic Disease Patients at Home.  It is well known that patients with chronic diseases have subtle exacerbations prior to clinical decompensation.  With this module, you can set the normal range of values for your chronic disease patient (weight, blood pressure, blood sugar, etc.). Your chronic disease patient enters his values through his PHR.  If the values are outside the range you set, you are notified in the Inbox.  Then you can intervene and prevent clinical exacerbations and hospitalizations. The practice is notified in the Inbox of patients who do not entered their data as scheduled.  This module involves patient participation, a goal of the PC-MH.
• Integrated Personal Health Record. (PHR).   The PHR  gives your patients access to their medical record, medical documents, and a means of contacting you.  The Personal Health Report lists all the tests and procedures the patient should have for health maintenance. With My Stats, a patient can record his medical data for you and him to follow.  The PC-MH encourages patient participation in their health care and acts like a virtual health coach.
• Patient Encounter Interface.  This module enables to you to document your patient encounters efficiently.  Here are some of its unique features.
  • History.  With its comprehensive patient generated medical history (PGMH), the patient is empowered to tell his story.  The PGMH uses adaptative questionnaire technology.  A general question is asked and the positive responses result in additional follow-up questions that a physician would ask.  This results in a complete and comprehensive history.  In a way, the PGMH is like an automated health coach.  The physician can spend the time during the clinic visit counseling the patient based on the patient’s responses rather than interrogating the patient.   This transfers the workload of doing a history from you and your staff to your patients.  The PGMH enhances patient participation in his health care.  The PGMH is the portal for an e-visit and phone consultation.
  • Physical Exam.  There are no rigid templates.  Documenting physical findings is easy, just point and click.
  • Assessment and Power.  The assessment and plan is a working problem list of a patient's medical conditions and minimizes your workload and increases your efficiency.  Evidence-based treatment orders can be applied using MyFavorites.
  • ManageRx - A medication administration and prescribing system. The fast and easy way to manage your patients' medications. MNS ManageRx lets you decide which medications can be refilled and by whom. With this functionality, most prescriptions can be refilled by your staff without your involvement.
